A rare but deadly brain-eating amoeba is lurking in some U.S. tap water and freshwater lakes, posing a growing threat to unsuspecting swimmers and households. Naegleria fowleri, often called the “brain-eating amoeba,” has recently grabbed attention after a Missouri man died just days after waterskiing at the Lake of the Ozarks.
This microscopic parasite enters the body through the nose, travels to the brain, and causes a devastating infection known as primary amebic meningoencephalitis (PAM). With a fatality rate above 97%, this amoeba is as lethal as it is rare, and experts warn that climate change could make it more common. The Missouri case, reported in 2022, involved a man who contracted PAM while enjoying a day on the water. Within a week, he succumbed to the infection, which destroys brain tissue and triggers severe inflammation.
Health officials stress that Naegleria fowleri cannot infect people through drinking water or spread from person to person. It only becomes dangerous when contaminated water enters the nasal passages, such as during swimming, diving, or nasal rinsing with unsterilized tap water. Over the past 60 years, the U.S. has seen just 167 cases of PAM, but only four people have survived. Most victims are children or young adults, often males, who are exposed during summer water activities.
While Naegleria fowleri is typically found in warm freshwater environments like lakes, rivers, and hot springs, it has also been detected in treated tap water systems. In 2013, a Louisiana child died after using tap water in a water slide, marking the first documented PAM case linked to a U.S. public water system. More recently, in 2023, a Texas woman died after rinsing her sinuses with contaminated tap water from a campground. These cases highlight a rare but alarming risk: the amoeba can survive in pipes or water heaters, especially in warm climates or systems with low chlorine levels.
Climate change is making the situation worse. Rising water temperatures are expanding the amoeba range, with cases now reported in northern states like Minnesota and Indiana, which were once considered low-risk. Warmer waters and drought conditions concentrate the parasite, increasing the chance of exposure. As summer temperatures climb, more people flock to lakes and rivers, unknowingly raising their risk of encountering this deadly organism.
Prevention is straightforward but critical. When swimming in warm freshwater, experts recommend using nose clips or keeping your head above water to block the amoeba’s entry. Avoid stirring up sediment in shallow areas, where Naegleria fowleri often lurks. Only use sterile, distilled, or boiled (and cooled) water for nasal rinsing, as tap water may be unsafe. Public water systems are generally secure, but utilities may boost chlorination and flush pipes in areas with confirmed contamination. Residents should follow local advisories and avoid letting tap water enter their noses during bathing or showering.
Diagnosing PAM is challenging because symptoms like headache, fever, and confusion mimic bacterial meningitis. By the time doctors identify the amoeba, it’s often too late. Treatment involves a combination of drugs like amphotericin B and miltefosine, but success is rare. Early detection is crucial, and the Centers for Disease Control and Prevention (CDC) offers guidance for suspected cases. Survivors, like two children in 2013, owe their lives to rapid diagnosis and aggressive therapy, often including therapeutic hypothermia to reduce brain swelling.
