LL Cool J’s Unwavering Support for His Wife During Rare Bone Cancer Battle

LL Cool J and his wife Simone I. Smith have been married for more than 30 years, a milestone that stands out in an industry not always known for enduring relationships. Their bond was tested in 2004 when Smith was diagnosed with stage III chondrosarcoma, a rare and aggressive form of bone cancer, but the couple emerged stronger, with the rapper famously offering to donate his own fibula bone to aid her recovery.

The pair first met as teenagers in Queens, New York, around 1987 and dated for eight years before marrying on Aug. 7, 1995. Together, they have raised four children — Najee, Italia, Samaria and Nina — and now enjoy time with grandchildren while continuing to live in New York. Smith has often described their marriage with the phrase, “Marriage isn’t for the weak. It’s for the faithful,” emphasizing the work, love, listening and faith required to sustain a partnership through life’s challenges.

Smith’s diagnosis came after she noticed a small lump on her right leg, initially attributed to bumping it during a workout or while getting a leg wax. The cancer, which affects cartilage and commonly impacts long bones like the tibia, carried a challenging prognosis at the time, with five-year survival rates around 33% for stage III cases.

Throughout the ordeal, LL Cool J — born James Todd Smith — remained a constant presence. During medical consultations, when doctors discussed removing the affected bone and reconstructing her leg, the “Mama Said Knock You Out” rapper volunteered to donate his own fibula, believing it would be “bigger and stronger” for his wife. Though surgeons opted to use Smith’s left fibula instead, the gesture underscored his commitment.

Smith underwent a complex 15-hour microvascular reconstructive surgery in which her right tibia was removed and replaced with her left fibula, along with hardware including a steel rod, screws and nails. Recovery was lengthy and difficult, taking approximately 2.5 years before she could walk normally again, progressing from wheelchair to crutches, cane and independent mobility.

“[LL Cool J] wanted to give his fibula bone… because he felt that his fibula bone would be bigger and stronger,” Smith recalled in past interviews. She has credited her faith, family support and determination as a mother — “Ain’t nobody going to raise these kids with [LL] but me” — for helping her through the pain and rehabilitation.

More than 20 years later, Smith remains cancer-free and has become a prominent advocate. She serves as an ambassador for the American Cancer Society and partnered with LL Cool J and others on the “Beat Cancer Like a Boss” campaign. She also launched her own jewelry line, Simone I. Smith Jewelry, with designs inspired by her journey, including a signature lollipop pendant referencing a tattoo affected by her surgery. Portions of proceeds support cancer initiatives.

The experience not only transformed Smith’s perspective — she speaks of waking up daily with gratitude and not taking life for granted — but also deepened the couple’s connection. LL Cool J has emphasized the importance of staying present and maintaining positive energy during difficult times, calling the battle a testament to not abandoning loved ones in their hour of need.

As they celebrate over three decades of marriage, LL Cool J and Simone Smith continue to share their story publicly, offering inspiration to others facing serious illness. Their journey remains one of the more compelling examples of resilience, loyalty and partnership in the entertainment world.
